What happens at LTF clinics?

Prior to attending the LTF clinic, families are contacted by phone and asked a series of questions about the child or young persons current health and well-being. These questions will enable the team to assess their needs and ensure the child/younger person and family see the health care professionals relevant to them.
At each clinic appointment children and young people can expect to receive;

- a comprehensive physical, psychosocial and emotional assessment
- discussion about planned investigations and interventions (roadmap)
- an updated treatment summary

- specific treatment-related health links or information about actual or potential health issues arising from previous treatment

| The focus of the LTF clinical service is health promotion and awareness, early detection and intervention of cancer-related and other health and well-being issues. The emphasis is on empowering individuals and families to take control of their survivorship. Our aim is to improve future health possibilities and to prepare children, young people and families for transition into a suitable adult health care setting at the appropriate time.
The family local doctor (GP) will be kept informed about medical management and will be actively involved in ongoing care.
