| As part of the Victoria’s Cancer Action Plan 2008-2011 (VCAP), Long Term Follow-up (LTF) has been recognised as an area of high importance. The PICS has been funded to develop a statewide program for survivors of childhood cancer. The program will be accessible to all paediatric and young adult survivors of childhood cancer living in Victoria. It will provide each individual with a tailored approach to meet their ongoing needs, in the environment best suited to the level of care required and with an appropriate transition plan to adult-based community or hospital services. The LTF program clinical service uses a risk stratified approach that is dependent on the child/young persons diagnosis and the course of treatment. At the end of treatment and as children, young people and their families move to the LTF phase, the LTF team together with patients and families and the primary treating Oncologist, will establish the level of LTF care required and where it should be accessed (hospital, regional centre, GP or community paediatrician). PICS LTF clinics are held at the Children's Cancer Centres at RCH and Monash Children's at Southern Health and at designated regional centres.
