| The Department of Health (DH) defines care coordination as a comprehensive approach to achieving continuity of care for patients, seeking to ensure that care is delivered in a logical, connected and timely manner so that all medical and personal needs are met. In the context of cancer, care coordination encompasses multiple aspects of cancer care delivery including multidisciplinary team meetings, psychosocial assessment and the provision of required care, referral practices, data collection, development of common protocols, information provision and individual clinical treatment. The PICS is committed to continually reviewing and improving care coordination for children with cancer. For example, the PICS has developed the Regional Outreach and Shared Care Program (ROSCP). The ROSCP is a program that facilitates care coordination and transitioning between primary treating hospitals and the regional centres caring for children and adolescents with cancer. The program provides opportunities for shared care and ensures there are agreed levels of training and standards of care for medical, nursing, allied health and supportive services. |
